Band 5 ICS Staff Nurse

Leicestershire Partnership NHS Trust provides a range of community health services for both physical and mental health conditions, as well as a range of specialist services supporting people of all ages including family and young people services, mental health, learning disabilities and physical health community and inpatient services. We deliver services to the people of Leicester, Leicestershire and Rutland. Our 5500 staff serves a population of one million from over 100 sites across the region.
Intensive Community Support (ICS) offers an alternative to traditional hospital care for up to 256 adults requiring intensive nursing and therapy input within their own home. These virtual beds are split across five teams within Leicester City, Leicestershire and Rutland.
The ICS team comprises Registered Nurses, Occupational Therapists and Physiotherapists with support from Health Care Assistants, Technical Instructors and Administration staff. The ICS team also works closely with Local Authority Social Services and our Community Hospital Teams.
The post holders will participate in the delivery of skilled nursing care to patients within their own home and assist the patient to develop self-management skills.
- As an applicant you must demonstrate a strong commitment to working in an integrated manner within the community setting and supporting multi-disciplinary team development
- You will also have a proven ability to promote patient independence, a sound theoretical knowledge under pinning highly skilled assessment and nursing skills, demonstrate a flexible approach to patient care and be adaptable to new initiatives in a culturally diverse setting
